Please send the following when referring new patients:

• Comprehensive Diagnostic Evaluation (CDE) to establish individual's Autism Spectrum Disorder Diagnosis
•  Service Order by a Licensed Medical Doctor (MD), Licensed Doctor of Osteopathic Medicine (OD), or Licensed Psychologist according to their scope of practice shall complete and sign a service order. Each service order must be signed and dated by the authorizing professional and must indicate the date on which the service was ordered. A service order must be in place prior to or on the day that the service is initially provided. The service order must be based on a Behavioral, Adaptive, or Functional Assessment of the beneficiary’s needs.  Service orders are valid for one year. Medical necessity must be revised, and services must be ordered at least annually, based on the date of the original service order.  
• If applicable: most recent office notes with the patient

After you refer the patient to G.E.M.S, our Intake Team will contact the client within 1-2 business days, inquiring about prior history, medical details and more. Depending on the client's location, service needs in the home, community, or telehealth, and considerations, we will determine the acceptance of the referral and begin the intake process within 48 business hours.
